Injuries once again sabotaged Greene's season, as he hit the IL twice due to groin issues, totaling just 107.2 innings during the 2025 regular season before adding on an additional three innings in the wild-card round. He pitched brilliantly when available, posting a 2.76 ERA and a career-best 0.94 WHIP to go along with a 132:26 K:BB and only two hit batsmen after plunking an MLB-high 19 the year prior. The home-run rate jumped back up to 1.25 HR/9 after he made major strides there in 2024, suggesting the 2024 campaign will end up being the outlier in that regard. Still, despite the groin issues, Greene continued to showcase elite velocity for a starting pitcher, averaging 99.5 mph on his four-seamer, along with excellent swing-and-miss ability, ranking in the 89th percentile or above in Chase%, Whiff% and K%. His home park in Cincinnati doesn't help with the occasional bouts of gopheritis, and he's still mostly fastball-slider, but Greene has now performed at a near-Cy Young level for two consecutive seasons, making good on his prospect promise. Elbow issue dates back to last year
Greene revealed Wednesday that the right elbow soreness which prompted an MRI dates back to his "final 5-6 starts" of last season, Charlie Goldsmith of Fox 19 Now Cincinnati reports.

Greene said of the elbow soreness last season that "it was a playoff push, so I pushed through it." The hurler noted that he got an injection and "felt fine" during the offseason, but the discomfort returned a week before the start of spring training. Greene made a Cactus League start Feb. 28 and was touched up for four runs on five hits and one walk over one inning of work. The righty is being sent for an MRI, which will be evaluated by team physician Dr. Timothy Kremchek before being sent to specialist Dr. Neal ElAttrache for a second opinion. Greene said that "I know I have bone spurs" in the elbow, but "as of now, there's no UCL damage." Those comments were made prior to the scheduled MRI, so more will be known about Greene's outlook after he's evaluated by both doctors. At minimum, he will be shut down through at least next Tuesday.

Right elbow soreness put Greene on the injured list in mid-August and derailed his first All-Star campaign. He was able to return for two abbreviated starts at the end of September, which helped restore confidence in his health heading into the offseason. In total, Greene tossed a career-high 150.1 innings for the Reds and pitched to a 2.75 ERA, fourth lowest among pitchers with at least 150 frames. His 1.02 WHIP was seventh lowest among that group. Primarily a fastball-slider pitcher, Greene introduced a splitter and curveball while bagging his changeup last year. His strikeout rate dipped a bit, but it was still impressive at 27.7%, and he made huge strides with his home-run rate, cutting it by more than half to 0.72 HR/9. The second overall pick in 2017 is making good on his prospect promise. Now 25, Greene is right on the verge of ace status, with the ace workload representing his final hurdle.
