2025 Fantasy Outlook
Injuries have limited Milano to a mere nine regular-season appearances over the last two years, making his 2022 All-Pro campaign feel like a distant memory. Now heading into his age-31 campaign with one year remaining on his deal with Buffalo, Milano essentially finds himself in 'prove-it' territory. He remains a lock for a starting role in the Bills' linebacker corps as long as he remains healthy, but given his recent trajectory, it's possible Milano never again approaches a 100-tackle season. Won't practice Wednesday

January 7, 2026
Milano (illness) will not practice Wednesday, Maddy Glab of the Bills' official site reports.
ANALYSIS
Milano played in the Bills' last nine games consecutively before resting with most of the defensive starters in the team's Week 18 victory over the Jets. The linebacker appears to have acquired an illness, but it will likely not stop him from playing in Sunday's playoff game against the Jaguars. The 31-year-old will have two more opportunities to elevate his practice participation and prove he is over the illness in time for Sunday's contest.

Milano, who was a first-team All-Pro just two seasons ago, has a degree of upside as a fantasy asset and has proven his value as a steady NFL defender for seven seasons now. As he approached his age-30 campaign, the 2017-third-rounder was returning from a leg injury that limited his effectiveness in 2023. Unfortunately 6-0, 223-pounder sustained another injury in August (torn biceps) that will sideline him indefinitely.
Milano is as steady as they come and still in his prime heading into his age-29 campaign. The 6-0, 223-pounder enters his seventh year with the Bills coming off a 2022 effort in which he recorded 99 tackles (72 solo), 1.5 sacks, 11 passes defended and three interceptions. This coming season, the Bills are once again counting on Milano to anchor a strong defense while working as a core piece at outside linebacker.
A solid all-purpose linebacker, Milano - who logged 86 tackles in 16 games last season - does a little bit of everything. Over the past two years he's had a bit more success rushing the passer with 6.5 sacks versus the 2.5 he recorded through his first three years in the league. This coming season, he'll reprise his starting inside linebacker role alongside Tremaine Edmunds
Milano signed a four-year contract extension this offseason, keeping him with the Bills, who made him a fifth-round draft pick in 2017. He's coming off of a season where he battled injuries, appearing in only ten games with just five starts, but the linebacker posted 101 total tackles in 2019 - when he was mostly healthy - and did record 3.5 sacks and an interception in more limited action this past season.
The fourth-year linebacker has improved his production in each of his three NFL seasons, and if that continues, he'll have a strong 2020 campaign. Last year Milano improved from 69 tackles, one sack, and seven passes defended in 2018 to 92, 1.5 and nine. This will be the final year of his rookie contract, so the three-down player has an opportunity to set himself up for a big payday.
Heading into his third campaign as a pro, Milano should be on deeper IDP radars after posting 78 tackles, seven passes defensed, three interceptions and three fumble recoveries in just 13 games for the Bills in 2018.
The second-year linebacker out of Boston College is well thought of by the Bills and has a chance to break out in 2018. One indicator is the 25 total tackles Milano produced over the final four games of his rookie season last year. It's easy enough math to extrapolate that out to 100 for a full campaign, which would be solid enough production for the 23-year-old to merit IDP consideration.

Beane optimistic about '25
LBBuffalo Bills
March 7, 2025
Bills general manager Brandon Beane stated at the NFL Combine that he was optimistic about Milano's 2025 outlook, Alaina Getzenberg of ESPN.com reports.
ANALYSIS
Milano has played in only nine regular-season games since the 2022 campaign due to an assortment of serious injuries. However, the 2017 fifth-round pick went into the offseason healthy and riding some momentum, as he was able to record 33 total tackles, including 2.0 sacks, and two fumble recoveries across four regular-season games and three postseason contests following his Week 13 season debut in 2024. "We expect Matt back at this point," Beane said. "I'm excited that he's healthy and not having to have a surgery or a rehab situation."
