Simmons has improved his ability as a pass rusher as his career has progressed, and that skill peaked in 2025 when he posted 11.0 sacks, a 19.4 percent pass-rush win rate, and 21 quarterback hits - all career bests by a wide margin. The Titans rewarded that breakout by extending him for three years on a deal worth $108 million this offseason, but the question will be whether Simmons can maintain the progress he made in 2025 or if he will revert to his prior career norms. The argument for the positive is that new head coach Robert Saleh is a well-respected defensive mind, and he'll almost certainly build a significant amount of his game plans through Simmons. On the other hand, Simmons' career norms suggest that his 2025 results were an outlier. The good news for fantasy managers is that even if he regresses, Simmons is a workhorse along the defensive line who is still a strong enough run defender to remain a borderline-elite IDP option, though the question will be just how high his ceiling is entering the 2026 season. Per Ian Rapoport of NFL Network, the three-year pact -- which makes Simmons the highest-paid defensive tackle in NFL history -- amounts to a $105.8 extension with $100 million guaranteed. The 2019 first-rounder -- who is bouncing back from an offseason elbow procedure -- is coming off a 2025 campaign in which he recorded 67 total tackles, including 11.0 sacks, in 15 regular-season contests.

Simmons was greatly limited by an array of leg injuries in 2023, but he regained his health in 2024. His sack total of 5.0 was a disappointment on the surface, but Simmons played a career-high 342 snaps in run defense. That allowed him to log a career-high 76 tackles. That shift in role was likely due to game script rather than a preferred scheme by the Titans, so a higher sack total and lower tackle total is likely in the cards for 2025. Regardless of exactly how it comes, Simmons will be productive in some form so long as he can remain on the field.
Simmons had a disappointing 2023 campaign after signing a four-year, $94 million extension in the offseason. He was limited to 12 games due to injury, though it's unclear if he was ever truly healthy. He had a procedure in April of 2023 to clean up an issue with his ankle, an issue which may have briefly popped up again in late October. One month later, he suffered a knee injury that ultimately derailed his season for good. After a normal offseason, the hope is that Simmons will enter both training camp and the regular season healthy, in which case he should be productive as the centerpiece of the Titans' defensive line.
Mike Vrabel's commitment to the 3-4 defense is admirable, but it's keeping Simmons from being the dominant pass rusher his physical tools say he should be. Even so, the 19th overall pick in the 2019 draft has 16 sacks in 32 games the last two seasons, impressive production for a 3-4 defensive end. He's also topped 50 tackles each of those two years, giving him a solid fantasy floor when the sacks aren't coming. The return of Harold Landry gives Simmons more help off the edge, potentially freeing him up to reach double-digit sacks for the first time.
As a 3-4 defensive end it will be tough for Simmons to keep pace with his 4-3 counterparts, though he’s the rare type of talent who can disrupt consistently from the interior even while facing double teams. That especially became true during Simmons’ breakout 2021 season, when he produced 8.5 sacks on 883 snaps after producing only five sacks in the prior 1,157 snaps.
Simmons was off to a fast start in his 2019 rookie year, producing 32 tackles and two sacks on just 318 snaps, but his per-snap production fell a bit in 2020, his age-23 season. Although Simmons' snap count more than doubled, his tackle total barely improved, and he produced just one additional sack on the extra 521 snaps. He played through ankle and knee troubles and went on the COVID-19 List. Now that he's heading into his third season, Simmons might retain his new snap volume while regaining some lost efficiency, hopefully setting the stage for a breakout.
Simmons made his debut Week 7 following his offseason ACL tear, and the Mississippi State product immediately made clear why the Titans spent the 19th overall selection despite that injury. Including the playoffs, he would go on to post 35 tackles and two sacks in 400 snaps. Now about a year and a half removed from his ACL tear and with Jurrell Casey shipped off to Denver, Simmons is a candidate to play 800 or more snaps in 2020. Simmons exceeded 60 tackles in each of his last two seasons at Mississippi State, both of them 13-game seasons, and he looks like a good bet to exceed that figure in his second NFL season, too.
