2026 Fantasy Outlook
There were some good things to take away from Alcantara's first full season back from Tommy John surgery. He made 31 starts and threw 174.2 innings, which probably exceeded the expectations of even the most optimistic prognosticators. Alcantara's velocity was also essentially all the way back, which, we were reminded by Spencer Strider, isn't always a given. The results, unfortunately, were not there for Alcantara for much of the season. He posted an 8.47 ERA and 1.67 WHIP in the first two months, then was up-and-down for his next 12 starts. Alcantara's final eight outings, though, looked like vintage Sandy, as he held a 2.68 ERA, 0.86 WHIP and 52:10 K:BB over 53.2 frames. The strong finish offers hope that Alcantara can return to peak form consistently in 2026, but it would be risky to draft him as if that's a given. Read Past Outlooks

Alcantara (3-4) took the loss against the Blue Jays on Tuesday, allowing eight runs on 10 hits and one walk while striking out four across 5.2 innings.
Analysis
Alcantara gave up two runs over the first five innings of Tuesday's start and was brought back out for the sixth to secure his eighth quality start of the season. He gave up a solo home run to the first batter he faced in the frame, Yohendrick Pinango, and that turned out to be a sign of things to come for the right-hander. After getting the next two Blue Jays out, Alcantara proceeded to give up another five runs, four of which came on a grand slam from Jesus Sanchez. Alcantara struggled with his control, especially toward the tail end of his start as he hit four batters across the fifth and sixth frames. He has given up at least six earned runs in three of his last five starts, ballooning his ERA to 4.66 ERA across 75.1 innings. He'll look to bounce back in his next start, which is slated for next week on the road against the Nationals.

Alcantara spent the year rehabbing from Tommy John surgery undergone in October 2023. The timing of the procedure limited the right-hander to bullpen sessions, though he began to face live hitters in mid-September. All the progress reports were encouraging, but when he takes the hill in the Grapefruit League, it will be more than16 months since Alcantara threw a ball in anger. Typically, the exercises involved in Tommy John rehab strengthen the area, allowing the pitcher to match, and often add, velocity. The concern is always command and control, which were Alcantara's main assets before the injury. Even with a 98-mph fastball, Alcantara's strikeout rate was above average, but not elite. Workload will also be an issue. Rostering Alcantara is a matter of risk tolerance and level of reliance. It's best to treat him as a luxury item and not a staff fixture.

Latest Fantasy Rumors
Likely staying in Miami
Marlins owner Bruce Sherman has told staff members he expects Alcantara to be with the team on Opening Day, Bob Nightengale of USA Today reports.
Analysis
The Marlins passed on dealing Alcantara at the 2025 trade deadline despite heavy interest, and while teams are inquiring about the right-hander again this offseason, it seems at this juncture that the 30-year-old is likely staying put. Alcantara had an up-and-down first season back from Tommy John surgery in 2025, posting a 5.36 ERA and 142:57 K:BB over 174.2 innings covering 31 starts, but he finished strong with a 3.33 ERA in the second half. His contract has an affordable $21 million club option for 2027, so the Marlins may be hoping they can get a better return for him if he continues that success in the first half of 2026.
